Ce formulaire permet d'envoyer une demande de rendez-vous dans ce centre d'abandon du tabagisme (CAT). Les informations demandées permettront à un intervenant de ce CAT de prendre contact avec vous ou la personne référée afin de faire le suivi de la demande.
