Residents outside Québec

  1. Complete and sign the Access to Information on Vaccines Request form;
    If you do not have a Régie d’assurance maladie du Québec (RAMQ) health insurance number, you do not need to specify one.
  2. Send the form, along with a photocopy of a valid I.D. document (health insurance card or birth certificate) to your regional officer, by one of these ways:

    • Fax: 418 266-7024.

Once your request has been received, the officer in charge of access and protection of personal information will send your request by mail. The response time is about 2 weeks following receipt of the request. This response time may be longer depending on the number of requests received.

Access to information on vaccines requests, and responses to them, will not be dealt with by telephone or in person.

Acknowledgements of receipt will not be sent for faxed or mailed requests.

If you discover that the information is incomplete, you may request access to information from each establishment that administered vaccines.
